Introduced by the Human Services and Finance Committees of the:

INGHAM COUNTY BOARD OF COMMISSIONERS

WHEREAS, Ingham County has a Health Department Medical Group Participation Agreement with Physicians Health Network; and

WHEREAS, Physicians Health Plan has advised Ingham County that it is necessary to amend the agreement to replace Exhibit 4, Commercial MSS Services billing codes and to replace Appendix D, Attachment G identifying the billing codes for MSS/ISS.

WHEREAS, the Health Officer recommends that the Agreement be amended to incorporate the new billing codes.

THEREFORE BE IT RESOLVED, that the Ingham County Board of Commissioners authorizes an amendment to the Health Department Medical Group Participation Agreement with Physicians Health Network.

BE IT FURTHER RESOLVED, that the amendment shall replace Exhibit 4 with a new Exhibit 4 identifying revised billing codes for Maternal Support Services and shall replace Attachment G of Appendix D with a new Attachment G identifying new billing codes for Maternal Support Services and Infant Support Services.

BE IT FURTHER RESOLVED, that all other terms and conditions of the agreement shall remain unchanged.

BE IT FURTHER RESOLVED, that the Board Chairperson is authorized to sign the amendment after review by the County Attorney.
