INGHAM COUNTY BOARD OF COMMISSIONERS

RESOLUTION TO AUTHORIZE A BUDGET ADJUSTMENT IN THE HEALTH DEPARTMENT AND THE PURCHASE OF COMPUTER EQUIPMENT AND SOFTWARE FOR THE IMMUNIZATION PROGRAM

RESOLUTION #01-87

WHEREAS, the County has a statutory responsibility to make mandated immunizations available to children; and

WHEREAS, the County has a contractual responsibility to record and report the status of childhood immunizations provided by the County and by other providers in the County; and

WHEREAS, the software that has been used for many years to record and report immunization status and events no longer is supported by the State of Michigan and no longer meets the needs of the Ingham County Immunization Program; and

WHEREAS, the staff of the Health Department's Communicable Disease Program and Immunization Program have worked with the County MIS Department and the State of Michigan to evaluate alternate computerized systems to record and report the immunization status of Ingham County residents; and

WHEREAS, the Health Officer has recommended that the Board of Commissioners authorize an amendment to the Health Department's 2001 budget and authorize the MIS Director make purchases and execute maintenance agreements to implement a new system of hardware and MedaTrac software from Sabre Consulting, Inc.

THEREFORE BE IT RESOLVED, that the Ingham County Board of Commissioners authorizes an adjustment to the Health Department's 2001 budget, to transfer $15,501 into the Office Equipment line item: 221-60120-99-979000-1220; and to transfer $1,000 from the Office Supplies line item: 221-60120-55-730000-1220 and $14,501 from the Biologics line item: 221-60120-55-764000-1220.

BE IT FURTHER RESOLVED, that the MIS Department Director is authorized to purchase computer equipment and software to house and operate the MedaTrac system, not to exceed $10,001.

BE IT FURTHER RESOLVED, that an agreement with Sabre Consulting, Inc. is authorized to purchase the license to operate MedaTrac, provide for annual maintenance and to facilitate the transfer of data for the County's existing VACS system to MedaTrac.

BE IT FURTHER RESOLVED, that the agreement with Sabre Consulting, Inc. shall provide for a one time payment of $5,000 by the County in the form of a software licensing fee to support 5 "full function" users and unlimited "view only" users, and a one time payment of $1,000 by the County to facilitate the transfer of immunization records from the County's existing VACS system to the MedaTrac, and for an annual maintenance payment by the County in the second year of 15% of the original purchase price of $5,000 and beginning in the third year of 15% of the current list price.

BE IT FURTHER RESOLVED, that the Controller is authorized to make the budget adjustments identified and the MIS Director is authorized to purchase the equipment and software authorized and the Board Chairperson is authorized to sign the agreement with Sabre Consulting, Inc., after review by the County Attorney.
