INGHAM COUNTY BOARD OF COMMISSIONERS

RESOLUTION TO AUTHORIZE A CONTRACT WITH PPOM, L.L.C.

RESOLUTION #98-155

WHEREAS, the Ingham County Health Department operates a network of community health centers; and

WHEREAS, the Health Centers currently serve about 50 handicapped adults employed by Peckham Vocational Industries, who have health insurance coverage through PPOM, L.L.C. (PPO Midwest), commonly referred to as PPOM; and

WHEREAS, the Department wishes to be paid for services when such services are covered by the health insurance product, and PPOM requires a contractual relationship with any providers in its network; and

WHEREAS, the Health Officer has recommended that the Board of Commissioners authorize an agreement with PPOM, L.L.C. and facilitate the process whereby the County can receive payment for health care services delivered to PPOM covered patients.

THEREFORE BE IT RESOLVED, that the Ingham County Board of Commissioners authorizes a contract with PPOM, L.L.C., Delaware limited liability company with headquarters at:

BE IT FURTHER RESOLVED, that the agreement shall provide for Ingham County to provide health care services to persons enrolled in PPOM products and for the County to be paid at negotiated rates for services covered in the PPOM products.

BE IT FURTHER RESOLVED, that the Board Chairperson is authorized to sign the agreement after review by the County Attorney.
