RESOLUTION TO AUTHORIZE A DIABETES OUTREACH NETWORK AGREEMENT

RESOLUTION #98-18

WHEREAS, the Ingham County Health Department provides services to persons with diabetes and persons at-risk for diabetes; and

WHEREAS, the Tendon Diabetes Outreach Network exist to identify patients with diabetes to provide them with diabetes care and education according to guidelines provided by the Network, and to document the health care delivery and process outcomes of providing care; and

WHEREAS, the Tendon Diabetes Outreach Network has offered to compensate the County for the identification of patients with diabetes and treatment of diabetes patients according to protocols and the reporting of identification and treatment efforts; and

WHEREAS, the treatment protocols are consistent with good medical practice and the Health Officer recommends that the Board of Commissioners authorize the agreement.

THEREFORE BE IT RESOLVED, that the Ingham County Board of Commissioners authorizes a diabetes outreach, identification, care and education agreement with the Tendon Diabetes Outreach Network, a program of Mercy Health Services.

BE IT FURTHER RESOLVED, that the period of the agreement shall be October 1, 1997 through September 30, 1998 and that the Network shall pay Ingham County $10 per Short Intake form, $15 per Long Intake form, and $10 for each Follow-Up form submitted, with the total revenue estimated at $1,200 during the agreement period.

BE IT FURTHER RESOLVED, that the Board Chairperson is authorized to sign the agreement after review by the County Attorney.
