RESOLUTION TO AUTHORIZE A LEASE AGREEMENT FOR THE LESLIE HEALTH CENTER

RESOLUTION #99-107

WHEREAS, the County leases 1314 square feet of space at 100-b South Main Street in Leslie for the purpose of operating a community health center; and

WHEREAS, the current five-year lease expires on April 30, 1999, and the property manager has agreed to a new five-year lease agreement at the rate of $5.02 per square foot per year; and

WHEREAS, it is important to maintain a community health center in Leslie, as the site provides access to primary care services (500 clients made about 1,400 visits in 1998) and categorical public health services, including immunization, family planning, STD diagnosis and treatment, WIC and blood pressure testing; and

WHEREAS, the Health Officer has recommended that the Board authorize and execute the proposed lease agreement.

THEREFORE BE IT RESOLVED, that the Ingham County Board of Commissioners authorizes a lease agreement with Maple Grove Properties for 1314 square feet of space at 100-b South Main Street, Leslie, Michigan.

BE IT FURTHER RESOLVED, that the agreement shall be in effect for the period May 1, 1999 through April 30, 2004, and that the County shall pay $5.02 per annum per square foot for the occupied space.

BE IT FURTHER RESOLVED, that the Board Chairperson is authorized to sign the lease agreement after review by the County Attorney.
