RESOLUTION AUTHORIZING ENTERING INTO AGREEMENTS FOR EMPLOYEE

LIFE AND DISABILITY INCOME BENEFIT PROGRAMS



RESOLUTION #99-130

WHEREAS, Ingham County provides life and disability income insurance to employees; and



WHEREAS, the Ingham County Health Coalition has conducted a review of the life and disability markets; and



WHEREAS, this process has resulted in a significant reduction in life and disability premiums.



THEREFORE BE IT RESOLVED, that the County enters into a new agreement with Canada Life for employee disability income coverage for a two-year period beginning on July 1, 1999 at a premium of $.12 per $100 of covered monthly payroll for both benefit levels which will result in an estimated annual premium of $36,300 for Ingham County employees.



BE IT FURTHER RESOLVED, that the Ingham County Road Commission, at their expense, be included as a subgroup in the disability income policy with Canada Life.



BE IT FURTHER RESOLVED, that the County enters into an agreement with Fortis Benefits for employee/retiree life insurance coverage for a two-year period beginning on July 1, 1999 at a premium of $.16 per $1,000 of life and $.02 per $1,000 of accidental death and dismemberment coverage which will result in an estimated annual premium of $51,304.



BE IT FURTHER RESOLVED, that the Board Chairperson and County Clerk are authorized to sign appropriate documents after review by the County Attorney.
