INGHAM COUNTY BOARD OF COMMISSIONERS

RESOLUTION TO AUTHORIZE CONTRACTS FOR DENTAL SERVICES

RESOLUTION #99-82

WHEREAS, the Ingham County Health Department operates a Dental Clinic which serves between 1,500 and 2,000 low-income persons annually; and

WHEREAS, this level of service requires approximately one and one-half dentist; and

WHEREAS, the contracts with dentists expired December 31, 1998 and Ingham County issued a RFP for dental services and four dentists responded; and

WHEREAS, the County issued an RFP for dental services; and

WHEREAS, the proposals that were received were all from dentists who were under contract with the County in 1998 and the Health Officer has recommended that contracts with these four dentists be offered for a two year period.

THEREFORE BE IT RESOLVED, that the Ingham County Board of Commissioners authorizes a contract for approximately 10 hours per week of dental services with Julia M. Gudmundsen, D.D.S., 4629 Grandwood Drive, Lansing, Michigan 48917, for the period January 1, 1999 through December 31, 2000, with compensation at the rate of $37 per hour during calendar year 1999 and $38.11 during calendar year 2000.

BE IT FURTHER RESOLVED, that the Ingham County Board of Commissioners authorizes a contract for approximately 16 hours per week of dental services with Dawn E. Fleming, D.D.S., 3328 Nonette Drive, Lansing, Michigan 48911, for the period January 1, 1999 through December 31, 2000, with compensation at the rate of $35 per hour during calendar year 1999 and $36.05 during calendar year 2000.

BE IT FURTHER RESOLVED, that the Ingham County Board of Commissioners authorizes a contract for approximately 24 hours per week of dental services with Jeffrey Bush, D.D.S., 1510 Deborah Drive, Apartment #216, Lowell, Michigan 49331, for the period January 1, 1999 through December 31, 2000, with compensation at the rate of $35 per hour during calendar year 1999 and $36.05 during calendar year 2000.

BE IT FURTHER RESOLVED, that the Ingham County Board of Commissioners authorizes a contract for approximately 10 hours per week of dental services with Matthew Song M. Yun, D.D.S., 241 West Broadway, Charlotte, Michigan 48813, for the period January 1, 1999 through December 31, 2000, with compensation at the rate of $35 per hour during calendar year 1999 and $36.05 during calendar year 2000.

BE IT FURTHER RESOLVED, that the Board Chairperson is authorized to sign the contracts for dental services after review by the County Attorney.
