certificate must sign before a Deputy Clerk in the County Clerk's Office
or in the presence of any notary public before filing.

a D.B.A., Co-partnership and/or a Dissolution Certificate is $10.00.
Two certified copies are provided at the time of filing. Additional
copies are $2.00.
