The seal of the Supreme Court (below) is a link which takes you to the web page maintained by the Michigan Supreme Court, where they have available a Small Claims Affidavit and Claim form (DC84). You can also obtain a Demand for Removal (DC87), Judgment (DC85) and Motion to Set Aside Default & Order (DC99) at this same location. You can complete these form on line and print to your own computer's printer, using a .PDF format.
The .PDF format requires you to have installed on your computer, Adobe Acobat Reader, which will allow your printer to print out the form as it appears on your computer screen. There is a link below from this page to the Adobe web site, so that you can download (free of charge) the Adobe Acobat Reader.
Forms are also available at the civil division counter at the court building. There is a nominal charge of $1.00 for each form requested.
